LivLong MD is a premier longevity and wellness clinic in Scottsdale, AZ, seeking a visionary PRN Physician Assistant (PA) or Nurse Practitioner (NP). You will co-develop, launch, and lead our new Medical Weight Loss and Hormone Replacement Therapy (HRT) programs. In this hybrid role, you will split time between clinical administrative development and on-site patient care. You will work in direct collaboration with our Medical Director to shape the future of preventative healthcare.

Job Summary:

Responsibilities:

Program Development: Design protocols for medical weight loss and HRT programs.
Clinical Leadership: Manage patient care pathways and clinical outcomes.
Patient Care: Conduct on-site evaluations, lab reviews, and consultations and manage evidence based ongoing weight loss and hormone management.
Collaboration: Partner with the Medical Director on compliance and safety.
Admin Management: Create patient onboarding materials and tracking systems.
Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories to inform personalized wellness plans.
Lab Management: Order, track, and thoroughly review comprehensive laboratory panels (including hormones, metabolic markers, and lipids) to establish baselines and monitor ongoing safety.
Provide health coaching focused on lifestyle modifications, nutrition, occupational health, and preventive care strategies to optimize longevity outcomes.

Qualifications:

Licensure: Active, unrestricted NP or PA license in Arizona.
Credentials: Active DEA registration and NPI number required.
Experience: Minimum 2 years of direct experience with comprehensive HRT and medical weight loss.
Modality Expertise: Proficient with GLP-1 protocols and all types of HRT (including pellets, topicals, and injectables).
Background: Proven track record in longevity, functional, or anti-aging medicine.
Skills: Strong leadership, clinical autonomy, and experience building programs from the ground up.

Required Clinical & Administrative Experience:

Medical Weight Loss & Metabolism Management-

GLP-1 Management: Proven experience prescribing, dosing, and managing patients on GLP-1 receptor agonists (e.g., Semaglutide, Tirzepatide).
Side Effect Mitigation: Documented history of managing common side effects, tracking lean muscle mass, and providing nutritional counseling.
Comprehensive Assessment: Expertise in evaluating metabolic markers, thyroid function, and insulin resistance to design personalized lifestyle programs.

Comprehensive Hormone Replacement Therapy (HRT)-

Diverse Delivery Methods: Experience managing patients on multiple HRT modalities, including topical gels/creams, oral therapies, and intramuscular injectables.
Hormone Pellet Therapy: Hands-on proficiency in performing subcutaneous hormone pellet insertions for both male and female patients.
Lab Interpretation: Advanced skill in interpreting complex hormone panels (e.g., free/total testosterone, estradiol, progesterone, SHBG) to precisely optimize candidate therapy.

Program Development & Administrative Leadership-

Protocol Creation: Prior experience designing, implementing, or updating clinical protocols, consent forms, and patient onboarding packets.
Collaborative Practice: Demonstrated success working alongside a Medical Director or practice owner to launch new service lines.
Clinical Autonomy: Experience working independently in a fast-paced boutique, wellness, or functional medicine clinic environment.
